This might not be a httpd relative question but it's worth a try.. I was wondering if it's possible to block anonymous/open proxies in some way? Whether its through httpd, firewall, php or anything else?

The only way I'd know of is to check their hosts with an open proxy scanner and somehow integrating the results with LSWS...but that's somewhat bad form and people don't like being portscanned just for navigating to a website. I wouldn't go to your site if I was portscanned.

For this type of connection bias filter, the best way to do this to via your firewall.

I believe some firewalls accept the usage of RBL (real-time block lists) when validating incoming connections. Obviously, it would add additional load on your firewall but that's the place to do it.
